Position Purpose
The purpose of this position is to support YRP’s Community Engagement Strategy by engaging with the community and promoting wellness and awareness with a diverse group and vulnerable populations. This position is responsible for providing a critical eye on Persons in Crisis (PIC) and vulnerable person occurrences and triaging appropriate follow ups for people who may be experiencing psychosocial distress.
Major Duties & Responsibilities
Provide mental health guidance to YRP members, external stakeholders and clients.
Work in partnership with Community Engagement and Wellbeing Bureau to provide community linkages and psychoeducation as well as short term support and system navigation for frequent users of emergency services with complex needs.
Review workflow queues for PIC related occurrences and evaluate which require follow up and by whom.
Conduct telephone and in-person consultations with clients in order to connect clients with community support services; complete psycho-social and risk assessments with clients and liaise with community partners to complete referrals and intakes.
Maintain databases for Project Lifesaver and Vulnerable Persons Registry; Versadex reports, E-Parade, and external databases.
Maintain secure, confidential and accurate records of personal health information while ensuring proper application of privacy legislation is met.
Provide short term support when working with regular/repeat high risk or complex clients. Identify and assist in strategies to reduce calls for heavy users of services and participate in community meetings designed to assist hard to serve populations/individuals or those at high risk.
Provide ongoing support and guidance to YRP members regarding clients. Evaluate cases that meet the threshold for acutely elevated risk on various York Region Situation Tables and conduct a thorough background search of relevant information to present to the Table committee.
Act as a resource for the organization on issues related to mental health vulnerable persons, and collaborate with external agencies on internal and external program resources, in order to assess and manage appropriate interventions and follow-ups.
Participate in community meetings, events and committees to exchange information and represent a policing perspective to mental health issues in the community and to bring program awareness.
Responsible to ensure that related programs for HealthIM, VPR, PLS are being utilized to their full capacity.
Assist with the development of training and education strategies to raise awareness of Mental Health issues and the available resources both internally/externally. Develop/lead internal program initiatives to ensure consistency with YRP’s values and Business Plan goals.
Create presentations and marketing and promotional materials in partnership with Marketing, Communications & Brand related to mental health and vulnerable persons.
Attend conferences and training seminars to keep current with professional development requirements in accordance with the Ontario College of Social Workers and Social Service Workers and ensure best practice procedures are being maintained.
Responsible for providing technical and functional guidance to internal members and community stakeholders regarding mental health issues and programs and services within the community.
Required to follow safety and security procedures in performing the duties of the job. Required to report unsafe public behavior.
Works with confidential information where there is a responsibility to disclose information that is covered by privacy legislation (PHIPA/MFIPPA).

Experience
Skills
Knowledge of the mental health, addiction, social and community services system in York Region.
Communication and interpersonal skills to interact with members of all ranks within the organization; external agencies and clients and their caregivers/families to provide and exchange information related to resources and programs for the client.
Crisis management skills for individuals experiencing crisis.
Collaborative teamwork problem solving abilities to assess and manage appropriate interventions and follow-ups for the needs of the client in conjunction with community support programs.
Ability to handle a multitude of tasks and work in a fast-paced, demanding environment while ensuring accuracy and attention to detail.
Strong interpersonal, organizational and planning skills.
Strong decision-making abilities and analytical skills.
Computer and keyboarding skills, including use of the internet and Microsoft Office applications.
Experience and knowledge of the criminal justice system is an asset.
Experiencing working with police is an asset.
Comprehensive understanding of PHIPA, MFIPPA, and the Mental Health Act.
Valid G license.
